Difference Between “Concussion” and “TBI”

Frequently, mild traumatic brain injuries are treated like minor concussions in the emergency. Mild traumatic brain injuries do not show up on CT’s thus, it is not medical neglect. Too frequently very little immediate medical treatment is adminisitered unless you have an open head wound, coma or impairment of gross motor skills.  Yet, those that suffer TBI’s often experence months if not years of symptoms and permanency.

Brain Injury: The Basics

While you can initially be diagnosed with a concussion, the importance of monitoring your symptoms is critical.  A “concussion” can vary from inflamation to coma–big range. Often those injured in car accidents are focused on broken bones and bleeding.  Many just figure their head should hurt because they were in a violent crash.  The primary difference between a mild TBI and a TBI is the testing and imaging.  Insurance companies very much like to point out that x-rays and CT’s do not reflect a TBI.  Failing to show up on standard imaging is by definition a classifcation of mild-TBI.  The term “mild” does not mean the brain injury causes no life-changing problems.  The term “mild” simply means there is not a gross abnormality on the CT scan.  Traumatic brain injuries are apparent in CT’s.  Most often, those suffering from mid-traumatic brain injuries need a battery of testing including cognitive, physial exam, MRI’s and DTI testing to illustrate the extent of the brain injury.

Mild Traumatic Brain Injury

Do not let the name fool you: even mild TBI can have serious impacts on your overall life, including your ability to return to work or to handle normal activities and interactions. Symptoms of mild TBI include:

  • Increased fatigue or difficulty completing normal activities
  • Increased headaches, including headaches of unusual intensity
  • Sleep changes—either sleeping too much or the inability to fall asleep
  • Sensory changes, including changes in the sense of smell and taste, visual disturbances, or auditory changes
  • Mood changes
  • Slow thinking, including unusual difficulty solving normal problems
  • Dizziness
  • Loss of balance
  • Emotional disturbance, including increased anger or irritability, more instances of crying, or extreme emotional response to seemingly normal stimuli
  • Increased sensitivity to sensory input
  • Getting lost more often than normal
  • Difficulty responding to otherwise typical situations

Severe Traumatic Brain Injury

Severe traumatic brain injury involves more serious symptoms and often include open head trauma or penetrating brain injures, those more severe syptoms can include many more symptoms  including:

  • Severe, noticeable cognitive deficits in a variety of areas
  • Difficulty paying attention or keeping attention on tasks that usually bring pleasure and enjoyment
  • Ongoing confusion
  • Difficulty processing both sensory input and cognitive input
  • Ongoing sensory difficulties, including significant changes in the way the victim processes scents, noise, and sights
  • Memory problems, often including both short-term and long-term memory
  • The inability to speak or understand speech properly
  • Difficulty reading the written word or communicating via written word
  • Challenges with impulsiveness or a lack of emotional regulation
  • Mood swings
  • Blurred vision
  • Difficulty judging distances or other visual input
  • Seizures
  • Loss of stamina, including the inability to participate in activities that the victim once enjoyed
  • Increased aggression
  • Lack of self-awareness or awareness of what is going on around the individual

Symptoms of severe TBI may not resolve.  In some cases, the victim struggles permanent challenges.
